How to Join Our Diabetic Program

1. Call Carolina Apothecary and speak with a customer service representative who will take your name, mailing information and insurance information.
2. Mail or fax us your prescription from your primary care physician stating items to be dispensed and the frequency of testing.
3. We will send you an Assignment of Benefits form for you to sign and return to us in a postage paid envelope. This form allows us to bill your insurance on your behalf.
4. We will mail your supplies directly to your home.
5. We will bill Medicare or other insurance on your behalf. You will only pay your deductables and co-payments.
6. You will receive a courtesy phone call when it is time to reorder so you never have to worry about running out of testing supplies.
